Details Tab

The first tab to configure in creating a Consent Form is the Details Tab. Fill in the necessary information in the section and fields shown below.

 

The Basic Details column consist of the following information:

  • Consent Form Name – Unique name for the consent form

  • Description – Brief description of the consent form

  • DS Type Name – This list displays the data subject types created. Only one Data Subject Type can be associated with the Form and it is a required field. Data Subject Type essentially defines for whom the consent is collected using this consent form.

  • Identifier Group – This list displays the identifier groups for the DS Type selected. At least one group must be selected. The identifier fields in the groups selected will be added automatically to the consent form as required fields. At least one group of identifiers must have values when consent is submitted so that CPM can identify the person providing the consent.

  • Default Language – This is the default language of the consent form.

  • Program – Each Form is associated with a program (one and only one program). However, multiple forms can be associated with the same program.

  • Consent Validity – This allows you to select a time period for consent expiration. This field has the following drop-down options:

    • Based on conditions – The consent validity will be based on the country field with a jurisdiction identifier, given the country selected is present in the Consent Validity table.
    • Does not expire – This consent will have no expiration.
    • Custom validity (in days) – The consent validity set will apply to the form regardless of the country found in the form. This is configured in the Consent Validity page.

The Display Details column consist of the following information:

  • Title – This is the title header of the Consent Form

  • “Submit” Button Text – The name attribute used to specify the Submit button

  • “Submit” Text Color – This field lets you specify the color of the Submit button

  • “Reset” Button Text – The name attribute used to specify the Reset button

  • “Submit” Button Background – This field lets you specify the color of the Submit button background

  • “Reset” Button Background – This field lets you specify the color of the Reset button background

  • Use TrustArc Captcha – This checkbox enables you to add a captcha that displays when you render your consent form.

Under Brands column, click the Add Brand button to associate a brand with the consent form. You can associate multiple brands to the consent form. At least one brand must be associated with the form. A particular brand can be associated with multiple consent forms.
