The Questions & Responses Report.xlsx Excel template lets you view every response to every question across all assessments based on the same template — all on a single chart.
As shown in the screenshot above, question numbers run left to right across the top (1), while assessment names are listed top to bottom on the left (2).
Vertically alternating colors highlight different sections, with section names shown above the question numbers. The full text of each question appears below its number.
Answers with issues are shown in bold red. Answers with an assigned risk score also have a highlighted background on a 1–5 scale:
