Users with admin role privileges can add a custom page to a business process record template where up to 10 custom questions and answer fields can be created. Custom Question and Answer fields can also be added to the Business Process Summary Report.
Customizing the Template

From the left side of the page, hover the mouse over the Data Mapping & Risk Manager icon, and then navigate to Settings (1) > Record Templates (2).

Click the template name (3) or the Edit icon (4) in the Actions column.

Complete the following, and then click Next (7).
›Rename the business process template (5).
›Enter a description (6). This is optional.

Under Setup, change the custom page name (8) if needed.
📋 Note: The name in the Custom Page Name field becomes the title of the custom page on any business process records that use this template.

To add more questions to the custom page, drag and drop field types from the Choose Field Types column onto the main screen.

Customize question, description, and response fields by clicking the Edit icon (9) on the question field box.

›Use the drag-and-drop icon (10) on the question field to reposition it on the page.
›Toggle Enable Custom Question Page (11) on to make the custom page visible in business process records. If this toggle is off, the page will not appear in any record.
›Click Show all linked business processes or forms (12) to see the linked records and forms for this template.

Click Next to proceed to the Tags tab. Select the tags to associate with the custom page.

Click Finish. To view the custom page, open a business process record associated with this template.
Adding Custom Q&A Fields to the Business Process Summary Report
You can include the Custom Question and Answer fields in the Business Process Summary Report.

Click the ellipsis menu to the right of the business process record and select Business Process Summary Report.

In the Generate Report modal, check the Custom Page Q&A checkbox to include the Custom Question and Answer fields, and then click Run Report.
