Supplemental Form Name – The name to be used for the supplemental form.
Language – This is the default language of the supplemental form. Based on the language selected, labels for supplemental form fields will change. However, these labels can be customized. Translations for 65 languages are supported.
Custom Brand – The name attribute used to specify the brand name.
Display Details
Display Company Name – When this checkbox is selected, the company name is displayed in the intake form.
Display Default Company Logo – When this checkbox is selected, the default company logo stored in the system will be used in the supplemental form. Admins can also provide a URL for the logo to use in intake forms.
Header Name – This is the name that Admins want to display on top of the supplemental form.
Submit Button Name – The name attribute used to specify the Submit button.
"Submit" Text Color – This field enables you to specify the color of the Submit button.
Reset Button Name – The name attribute used to specify the Reset button.
"Reset" Text Color – This field enables you to specify the color of the Reset button.
"Submit" Button Background – This field enables you to specify the color of the Submit button background.
"Reset" Button Background – This field enables you to specify the color of the Reset button background.
